Weight-based problems add one more relationship to organize. The goal is not to memorize a longer formula—it is to keep the weight unit, dose unit, and requested answer aligned.

The relationship

Start with the unit you need.

Dose per unit weight × patient weight = required dose

If pounds are provided but the dosing relationship uses kilograms, convert the weight before applying the dose rate.

Performance rule

Do not trust arithmetic without a check.

Before you move on, ask whether the unit, direction, and magnitude make sense. A correct-looking calculator result can still come from the wrong relationship.

Worked example

See the structure before the speed.

A hypothetical educational order is 0.1 mg/kg for an 18 kg patient. What dose does the relationship produce?

  1. Dose rate = 0.1 mg/kg
  2. Weight = 18 kg
  3. 0.1 mg/kg × 18 kg
  4. kg cancels → 1.8 mg

Quick check: At 0.1 mg for each kilogram, 10 kg would produce 1 mg. Eighteen kilograms producing 1.8 mg is proportional.
